Referrals

If you would like to refer a patient for sub-fertility or IVF treatment there are two routes into the Unit.

Patients who meet the funding criteria set by your local CCG:

For patients who meet your current CCG's criteria for funded treatment, you will need to refer them into the subfertility clinic within the Women and Children's Hospital, Hull Royal Infirmary using the correct referral to the subfertility clinic form.  If you require additional forms please contact the subfertility clinic who will provide you with the required forms.

Please contact your CCG if you require information regarding the up to date funding criteria or contact the sub-fertility clinic within the Women and Children's Hospital, Hull Royal Infirmary.

The Hull & East Yorkshire Trust will not send your patient an appointment for the subfertility clinic unless the primary care investigations required by your CCG are fufilled.

Whereas simple fertiity treatment is available to all couples with a female BMI <35, funded IVF type treatment will only be available to couples who meet the funding criteria of individual CCGs.

Most CCGs require couples to have been trying for 2 years or more, however in circumstances where there is an obvious reason (eg no sperm, severe endometriosis, blocked fallopian tubes) then treament can start sooner.

CCGs will not routinely fund any couple who have any living children in their current or previous relationships, including adopted children (but not fostered). Neither partner should not previously have been sterilised

Hull CCG, East Riding CCG, North Lincolnshire, North East Lincolnshire

Female BMI <30, female age 23-42 yrs

Semen evaluation result

Serum FSH, LH, Prolactin day 3-5 of cycle

Day 21 progesterone

Serum Rubella and Chlamydia antibodies

Result of last Cervical Smear

Scarborough and Ryedale CCG

BMI <35 for6 months prior to referral, female age 18-42 yrs

Both partners proven non-smokers 6 months prior to referral

GP needs to apply to Individual Funding Request (IFR) Panel for approval to refer for IVF

Note: GPs may choose to refer to the Hull and East Yorkshire NHS Trust for secondary care investigations, by the same team who perform IVF. If IVF is found to be the necessary intervention, then the GP will need to apply IFR funding.

Vale of York CCG

BMI 19-29 for 6 months prior to referral, female age 23-40yrs

Both partners proven non-smokers 6 months prior to referral

GP needs to apply to Individual Funding Request Panel for approval to refer for IVF

Note: GPs may choose to refer to the Hull and East Yorkshire NHS Trust for secondary care investigations, by the same team who perform IVF. If IVF is found to be the necessary intervention, then the GP will need to apply IFR funding.

 

HM Forces

At least one partner to be registered with a Defence Medical Services Practice

BMI Male <30 and Female BMI 19-30  Female age <43yrs of age at time of treatment

Both partners confirmed non smokers 

 

 

Self funding patients: 

If the patient does not meet the funding criteria or wishes to self fund their treatment, please refer directly to the Hull IVF Unit.  

Once a referral letter has been received by the office administration staff, an appointment will be allocated and the patient will be contacted with the date and time of the appointment, along with all other relevant information they will need to know for their initial consultation. Referrals can be faxed directly into the department on 01482 382672.
